Biographical Info
Access Community Health provides a range of health and disability support services in the community:
- Personal care
- Household support
- Disability support services
- Nursing services including post-operative care, wound care, palliative care
- Serious injury support including spinal cord injury, brain injury
Where? Across Taranaki and Aotearoa. Support available in-person in your home.
When? Available 24 hours a day, 7 days a week to meet your needs.
Who? All ages
Ask for a referral to Access from your nurse or GP, Te Whatu Ora | Health New Zealand, ACC or your local Needs Assessment and Service Coordinator (NASC). Or you can refer yourself and we can help you with the next steps.
